How to Apply for a Voluntary Member with PhilHealth

philhealthBeing a PhilHealth member is really beneficial as not just you, but also your beneficiaries can take advantage of medical assistance. PhilHealth is the health insurance company administered by the Philippine government. All employers in the country are required to provide this benefit to their employees to secure the health and protection of Filipinos.

However, even unemployed people can benefit from PhilHealth assistance. If you are self-employed, or unemployed as of time being, you can still be a member under the Individually Paying Member category. This is also referred to as Voluntary Member by others. If you want to be a voluntary Philhealth member, here are what you need to do.

Steps

  1. Go to the nearest PhilHealth branch in your area.
  2. Ask for and fill out the Philhealth Member Registration Form or PMRF. You can also download this online.
  3. Submit the following requirements:
  • Photocopy of NSO Birth Certificate, valid IDs such as TIN ID or driver’s license, or other documents that can prove your identity
  • Photocopy of same kind of documents above for the beneficiaries
  • Photocopy of Alien Certificate of Registration (for foreigners)
  1. Wait for the release of your PhilHealth Number.
  2. Pay the contributions.

Make sure that you keep a copy of your PhilHealth Number as this will be permanent. You can also apply online through their official website. PhilHealth membership application process is not difficult. You can also encourage your loved ones, who are not members yet, to apply.

    yung philhealth id ko po hindi ko na po makita. pero naka register na po ako dun noon. hindi ko po alam yung number ng philhealth ko. mag reregister pa po ba ako ulit dun? salamat po.

    Good evening po!
    Kumuha po ako ng Philhealth number,
    ang binigay po sa akin na form is MDR..pag katapos po nag bayad po ako sa bayan center!tanong ko po kailangan ko pa ba pasok po pa ang contribution ko?o kailangan ko pa kumuha ng form ulit?

    ako po ay philhealth member ng ako ay nagwowork pa noon sa Health Department Region 4. Andito na ako sa US as US citizen , can I still re activate my previous Philhealth membership and pay the monthly contribution? I still have my ID card. with me. My son in the Philippines wants me to include in his insurance but the the problem is my philhealth needed to be activated again. What shd. I do?

    it’s been 15 years since my last contribution of my philhealth, is it possible to contribute again? I am working in SIngapore for the past 8 years.

    dati na ako may philhealth. pero 5years na ako hindi nagbabayad ngaun. magbabago ba ulet ako ng philhealth no or itutuloy ko lang ung dating philhealth ko?

      Baka bayad yun sa contribution. 200/month so 600 kapag 3mos.

    employed po ako, pero voluntary and sinabi ko sa philhealth ko. okay lang ba yun pag yun ang binigay kong account sa employer ko? wala bng problema nun sa philhealth and sa company ko?

    i Just want to ask if pwede po bang maghulog sa Philhealth kahit na philhealth number lang ang inaplyan ko ? yung parang anytime pwede ako maghulog ng voluntary without filing any papers to become a voluntary payer?

      change mo status from employed to voluntary. Then you can continue your contribution.

      Hi Ghriese. If you have separated from your employer but you want to continue contributing to PhilHealth, you can choose to be a Self-Employed and Individually Paying Members. You should register under the Informal Economy and pay the premium at any PhilHealth Office or accredited payment center.

      Here are the rates:

      If the average monthly income over the last 12 months is P25,000 and below, the quarterly premium is P600.
      If the average monthly income over the last 12 months is above P25,000, quarterly premium is P900

      You may pay monthly, quarterly, semi-annually or annually.
